Renaissance Care Center has taken efforts to apply all infection control guidelines to every aspect of what we do, including the admission process. Although our process is different, we remain committed to working with families to make the transition of moving easy for new residents. Whether you are coming for short term rehab, or if you require long term care, keeping our residents safe from COVID-19 and maintaining a positive and healthy environment is our top priority.
When you are admitting from a hospital, we require a referral from the hospital case worker, along with necessary documents include a History & Physical, recent laboratory test results, therapy notes, MAR and nursing/ medical professional notes. All referrals are considered on a case-by-case basis.
Admission Health Assessments
Before moving in, residents will undergo a virtual or in-person health assessment by our Nursing team.
Before moving in, residents must have a COVID-19 test 3 days prior to the move in date.
The First 14 Days
During the first 14 days at Renaissance Care Center, residents will undergo a precautionary isolation period and will be quarantined in their room.
Therapy, if needed, will take place in the resident’s room for the first 14 days, during the precautionary isolation period. When and if a resident needs to leave their room a face covering will need to be worn. Following that, therapy will be conducted in our therapy gym. We will limit the number of people in the gym at one time to ensure proper physical distancing.
Care Plan Meetings
Care plan meetings will be arranged with our care team members by phone or video conference to review treatment and review the rehabilitation goals.
